Make a referral

Inpatient and outpatient referrals

Contact the admissions team at the closest SCI/D Center during business hours for questions about eligibility and for referrals.

They will need the following information:
  • Injury type
  • Care required
  • Veteran name
  • DOB
  • Social Security number
  • Date of injury onset
  • Current Veteran location
